Job Description

This position is open to all qualified applicants.Avita Community Partners is seeking the right candidate to serve as a full-time Community Living Support (CLS) CNA for I/DD Services. The goal of this position is to support individuals with developmental disabilities to become active and integrated citizens within their communities. This will be accomplished by helping the individual to identify their personal preferences and implementing a plan of support which honors these preferences.
 
Staff in this position will work in cooperation with clients, families, community members, support coordinators, and consultants as identified through the Person-Centered Plans of the Service process.Must be Certified Nursing Assistant within the state of Georgia, hold and minimum of a High School Diploma or General Education Degree, and demonstrate sufficient reading, writing and computer skills so as to be able to perform the duties.
 
Candidates must be able to work a flexible schedule of late afternoons, evenings and weekends.
 
Must be 21 years of age with a drivers license valid in the State of Georgia. Motor vehicle insurance as required by Georgia Law, and a good driving record is required. Applicants must not have six (6) or more points against their driver's license or a suspended or revoked driver's license within the last five (5) years.
 
A Pre-Employment Background will be conducted. This includes, but is not limited to, any of the following: Criminal (Both State and Federal Finger Printing), Driving, Educational and Employment record verifications/inquiries. Job Description

The Blairsville Developmental Disabilities Service Center is in need of a Developmental Disabilities Professional (DDP) - Team Leader

Responsible for daily functionality of the center's services. Will assist in the development of Individual Service Plans (ISP), monitor the implementation and delivery of ISP, Behavior Support Plan, and Crisis Plan and will complete necessary addendums. Will complete HRST, Functional Behavioral Analysis and provide direct support to Intellectual and  Developmental Disability clients. Oversees and supervises the work activities of instructional and support staff. Serves as management coordinator for staff support, staff in-service training and family and community support programs.

Must be willing and able to:
*have dependable transportation
*use your personal vehicle as often as needed to transport clients
*work alone with a group of ten developmentally disabled clients
*assist clients in the use of toilet/bathroom
*change diapers of adult clients
*visit clients in their homes
*assist physically disabled clients in and out of wheelchairs and vehicles
*assist disabled clients who cannot feed themselves independently
*Must complete required annual training.

 

Minimum Qualifications: (Training and Experience) 

Bachelor's degree in a relevant human services field AND three years of in field service, two of which were in a supervisory position.

***Must have valid Georgia driver’s license and no driver may have 6 or more points against his/her license within the last five years and no driver may have had a suspended or revoked license with the last five years.
***In accordance with Georgia's Drug Free Workplace Act, it has been determined that this position warrants pre-employment and random drug testing.

Complete all Developmental Disabilities Professional (DDP)  job duties as described in the job description and supervise the residential program with the oversight of the Team Leader.


Job Responsibilities:
*Assist individuals with Developmental Disabilities in basic daily living skills.
*Maintain a clean, healthy, and safe environment for clients.
*Observes and monitors client's behavior, charts and reports observations and incidents.
*Provides training of basic personal and social skills, simple academics, or work skills to clients in residence, Avita facility, or supported employment environment.
*May assist with meal preparation, ensuring or encouraging client group activity, providing transportation, and/or limited case management.
*Keep accurate documentation of client progress and services performed, reporting changes in client condition to manager or supervisor.
*Will facilitate client community involvement.
*Remain aware of changes in the behavior and physical condition of clients, arranging for needed services to assure appropriate care is obtained. 
*Carry out a person-centered Individual Service Plan (ISP)
*Assist with meal preparation, ensuring or encouraging client group activity, providing transportation, and/or limited case management.

Residential Coordinator duties:
• Functions as Residential Coordinator; duties include monitoring host homes, review medical logs and audit daily notes, assist host homes with technology, track trainings and monthly paperwork, complete quarterly evaluations, assist host homes as needed.
• Provide direct care to clients as needed
Organizational skills a must to keep up with many deadlines, be able to effectively communicate expectations, be able to create residential homes, be able to maintain boundaries with those they supervise.

Minimum Qualifications: (Training and Experience)  

Completion of a bachelor's degree in Social Work or a bachelor's degree in a relevant human services field AND one year of experience to individuals with Developmental Disabilities.

Job Description: This is a FT, 12 month position. Provides mental health services to Children and Adolescents in the Union County School system during the school year and will perform outpatient services through our county Outpatient center during school breaks. Includes individual counseling, diagnostic assessments, group therapy, family counseling and Individual counseling.

Minimum Qualifications: Unlicensed ($30,822.75/yr.):A Masters Degree in Social Work, Mental Health Counseling, Community Counseling, School Counseling, Psychology, or Marriage and Family Therapy from a CACREP, CSWE, CCPTP or COAMFTE accredited university AND be license eligible with the Georgia Composite Board of Professional Counselors, Social Workers and Marriage and Family Therapists allowing for an associate level license within six months of employment (Required). (Upon receipt of Associate licensure, employee is eligible for a 5% increase in pay)

OR Associate Licensure as an LAPC, LAMFT or LMSW (32,363.89)

OR Full Licensure as LPC, LMFT or LCSW *Salary is dependent on type of licensure and length of licensure LMFT - Licensure with less than one year of post licensure experience - 40,000 - Licensure with one year of post licensure experience - 42,000 - Licensure with three or more years of post-licensure - 45,000LPC/ LCSW - Licensure with less than one year of post licensure experience - 42,000 - Licensure with one year of post licensure experience - 44,000 - Licensure with three years of post-licensure experience - 47,000
